To obtain a copy of a Motor Vehicle Accident Report, you must mail a $10.00 check or money order made payable to the Port Authority of New York & New Jersey, for each copy, along with the following information:

Date and Time of Accident
The Port Authority facility where the accident occurred:

• Goethals Bridge
• Holland Tunnel
• John F. Kennedy Airport
• LaGuardia Airport
• Lincoln Tunnel
• New Jersey Marine Terminal
• Newark Airport
• Outerbridge Crossing
• Port Authority Bus Terminal

It is helpful if you also state the names of the parties involved in the Motor Vehicle Accident, the Motor Vehicle Accident Number, and the name and badge # of Police Officer.
